It is a typical Portuguese communal flag, with the coat of arms centred on a plain blue field.

Coat of arms

Shield Or with base wavy of Azure and Argent, two watermill's wheels Gules in fess, in central chief a windmill's wheel Sable rigged Azure. Mural crown Argent with three visible towers (village rank) and white scroll (Bénard Guedes style) with inscription in black initials "PÓVOA DE RIO DE MOINHOS".
Meaning:
The name of the commune means "settlement at the river of mills". The arms are thus canting.
