It is a typical Portuguese communal flag, with the coat of arms centred on a plain yellow field.

The arms are Gules a head affronty Proper bearded Or helmeted Argent and fixed Azure,in chief Marian crown Or at dexter and Maltese cross Argent at sinister, in base a fess wavy Argent charged with a bar wavy Azure. Mural crown Argent with three visible towers (commune rank) and white scroll reading in black upper case letters "FREGUESIA DA MAIA".
While the blazon prescribes golden beard and silvery helmet, this image shows black beard and golden colour for the lower part of the helmet. This is not a mistake Sérgio Horta would do, so I guess this is based on actual use or even disagreement between the official text and the official image. This human head stands most likely for Gonçalo Mendes da Maia, “o Lidador”, (1079?-1185), a military figure of early Portuguese History.

Coat of arms

Shield Gules with base wavy of Argent and Azure, a paly palm leaf Or flanked by a wheat ear Or at dexter and a corn cob Or at sinister, in chief a Maltese cross Argent in each corner. Mural crown Argent with three visible towers (village rank) and white scroll with inscription in black initials "GUEIFÃÃES - MAIA".
Meaning:
The palm leaf is alluding to the martyr St. Faustino, the local patron saint. The Maltese crosses are alluding to the Order of St. John of Jerusalem aka Order of Malta, its presence in the area and its role in evangelising and developing the region. The wheat ear is symbolizing fertility, the corn cob is alluding to agricultural abundance. The bar wavy is representing River Leça, the border of the commune.
